  Evaluating the effectiveness of psyllium husk in conjunction with loperamide in treatment of patients with fecal incontience

Fecal incontinence (FI) which is known as unintentional leakage of solid or liquid stool has a global prevalence and easily can impede the life quality of patients, yet most patients due to cultural or social stigmatization followed by emotional burden of FI including loss of self-esteem, self-confidence, and mortification will not reveal or converse openly about their FI- struggling symptoms. Conditions including bowel inflammatory disorders such as; diarrhea, anal sphincter trauma due to childbirth, obstetrical injuries, cancer experiences, accident- based trauma in addition to, abdominal surgeries, and nerve paralysis due to accident or degenerative disease could cause FI symptoms. FI is categorized in accordance with its type which comes in three classes known as passive, active, urge concerning different severity in frequency, in urgency and amount of leakage. Among clinical interventions, simple approaches entailing fiber supplementation, application of laxatives and anti-diarrhea drugs like loperamide are step wise with less adverse-effects which could be fused by other most sophisticated clinical applications including biofeedback therapy, rectal sensation and others. In this study, non-surgical intervention of psyllium husk was taken into consideration. The objective of this study was to investigate the effectiveness of psyllium Husk in conjunction with loperamide, in reducing FI symptoms -induced diarrhea or constipation thus, diminishing FI-severity, and its unpalatable impact on patients' quality of life. The design of the study was observational-based, conducted in a clinical setting targeting couple of hospitals and nursing homes located in country of Iraq. The participants were individuals who suffered from passive FI and the intervention consisted of administering loperamide as a baseline treatment, followed by the administration of psyllium as an adjunctive therapeutic agent. Participants. The results concluded that psyllium supplementation could improve FI severity and ease its detrimental impact on patients' quality of life. Indeed, the results demonstrated a statistically significant decrease in role, physical, social, sexual, emotional, and sleep limitation while a better lifestyle adaptation and clinical outcomes were observed (p<0.05) between anti-diarrhea drug of loperamide as opposed to loperamide in conjunction with psyllium husk fiber.
